Seeing your record online

You can see parts of your health record in your NHS account, or in SystmOnline.

To set either of them up, see managing your health online.

Tell us once you have registered if you would like to see your record online. We check your record before we turn this on, and we will text you when we have.

This can take a while, depending on demand and GP workload.

Asking for a copy of your record

You have the right to a copy of your health record. Read how to get your medical records on the NHS website.

Your Summary Care Record

Your record is held securely at the surgery.

You may need treatment somewhere else in the NHS, such as an emergency department or a minor injuries unit. Staff there can give you better care if they can see part of your record.

The Summary Care Record shares:

• your current medicines
• any allergies you have
• any bad reactions you have had to medicines
• more information, if you ask your GP to add it

It can be seen across the NHS in England, including in urgent care, community care and outpatient departments.

Only healthcare staff involved in your care can look at it. They will ask your permission first, unless they cannot ask you and there is a clinical reason to look.

Opting out

Ask reception for an opt-out form.

A parent or guardian can ask to opt out a child under 16. The GP makes the final decision, because of their duty of care to the child.

If your child is under 16 and old enough to understand, please talk to them about it.
